I used the milk-based formula found in the Nourishing Traditions nutritional cookbook by Sally Fallon and Mary G. Enig, Ph.D. This recipe can also be found on the Weston A. Price Foundation site at http://www.westonaprice.org/children/recipes.html. There are also goat's-milk and meat-based formula recipes, as well.
Before I was able to get raw milk for my baby formula, and had to use pasteurized, I used kefirized pasteurized milk, since kefir contains more nutrients in it than just plain pasteurized milk. Of course, I started using raw milk as soon as possible, but he did use the "kefir formula" for about two weeks first. He made some faces, as I am sure it seemed quite sour as he was switching from breast milk, and learning the bottle and the flavour of formula all at once, but he did eventually start drinking it once he realized the boob was no longer an option. (Something in my genetics--my milk always dries up at 6 months.) Just another option if you can't seem to get your hands on raw milk--not ideal, but better than nothing.
I got most of my ingredients for the formula from Radiant Life (their phone number is under the recipe.) Some were available from my local grocery store, like organic olive oil and infant acidopholus. I got Garden of Life coconut oil from The Vitamin Bin. Check around and see what you can find out.
